Sugar advice tends to collapse into two unhelpful extremes: sugar is poison, or a calorie is a calorie so it does not matter. The actual guidance from health authorities is narrower and more useful than either, and it hinges on a distinction most people skip past — the difference between added sugar and the sugar that comes packaged inside whole foods.
The numbers
The World Health Organization recommends that free sugars make up less than 10 percent of daily energy intake, with additional benefits below 5 percent. For an average adult eating around 2,000 calories, 10 percent is about 50 grams, and 5 percent is about 25 grams — roughly 12 and 6 teaspoons respectively.
The American Heart Association is stricter, suggesting no more than about 25 grams a day for women and 36 grams for men.
"Free sugars" means sugars added by manufacturers, cooks, or you, plus sugars naturally present in honey, syrups, and fruit juices. It does not include the sugars inside whole fruit, vegetables, or plain milk.
A useful conversion for label reading: 4 grams of sugar is about one teaspoon.
Against those numbers, a single can of regular soft drink contains around 35 to 40 grams — meeting or exceeding a full day's allowance on its own.
Why added sugar is treated differently
The molecules are identical. The sucrose in an apple and the sucrose in a biscuit are the same compound. What differs is everything surrounding it.
Fibre changes absorption. In whole fruit, sugars are held within a fibrous cell structure that must be broken down before they are released. This slows digestion and blunts the resulting blood glucose rise. Blend or juice that same fruit and you destroy the structure — juice raises blood glucose substantially faster than the fruit it came from.
Volume and satiety. It takes real effort to eat four oranges. Drinking the juice of four oranges takes twenty seconds and satisfies you far less, because liquid calories register poorly with the body's appetite systems.
Nutrient density. Whole fruit brings fibre, potassium, vitamin C, and polyphenols. A sugar-sweetened drink brings sugar.
This is why large observational studies consistently associate whole fruit intake with lower risk of type 2 diabetes, while fruit juice trends in the opposite direction, despite similar sugar content.
Where the sugar actually is
If you asked most people to name their sugar sources, they would say desserts. In most national dietary surveys, the largest single contributor is sugar-sweetened beverages — soft drinks, energy drinks, sweetened coffee and tea, sports drinks, and juice.
Beyond drinks, the sugar in an ordinary diet is often in places people do not think of as sweet:
- Breakfast cereals and granola, including many marketed as healthy
- Flavoured yoghurt, which can contain 15 to 20 grams in a small pot
- Pasta sauces, ketchup, barbecue sauce, and salad dressings
- Bread, particularly soft sandwich loaves
- Protein bars and cereal bars
- Instant oatmeal sachets and flavoured milk
- Ready meals and stir-fry sauces
Reading three or four labels of things you buy weekly is usually more revealing than any general advice.
Reading the label
Two habits do most of the work.
Divide grams by four to convert sugar into teaspoons. It makes the number concrete in a way grams do not — "18 grams" is abstract, "four and a half teaspoons in one small yoghurt" is not.
Check the ingredients list, which is ordered by weight. If a sugar appears in the first three ingredients, the product is essentially a confection. Manufacturers sometimes split sugar across several names so no single one ranks high. Names to recognise:
- Anything ending in -ose: sucrose, glucose, fructose, dextrose, maltose
- Syrups: corn syrup, high-fructose corn syrup, rice syrup, malt syrup, golden syrup
- Concentrates: fruit juice concentrate, cane juice
- The "natural" ones: honey, agave nectar, maple syrup, coconut sugar, molasses
That last group deserves emphasis. Honey, agave, and coconut sugar are marketed as healthier alternatives, and they contain trace minerals white sugar lacks — in quantities far too small to matter nutritionally. Metabolically they are added sugar, and all major guidelines count them as such.
What excess sugar actually does
The evidence is strongest and clearest for a few specific things, and weaker for the sweeping claims.
Well supported:
- Tooth decay. This is the most direct and least disputed harm, and it depends on frequency of exposure as much as total quantity — sipping a sweet drink over an hour is worse than drinking it quickly.
- Weight gain, primarily through liquid sugar, which adds calories without proportionate fullness.
- Type 2 diabetes risk, most strongly associated with sugar-sweetened beverages.
- Displacement. Sugar-heavy foods tend to crowd out more nutritious ones, particularly in children's diets.
Plausible but more debated:
- Direct effects of high fructose intake on liver fat, independent of overall calories. The mechanism is real; how much it matters at typical intakes is still argued.
- Cardiovascular effects independent of body weight.
Not well supported:
- Sugar causing hyperactivity in children. Multiple controlled trials, including ones where parents were told their child had received sugar when they had not, have failed to find the effect. Parental expectation predicted the reported behaviour better than the sugar did.
- Sugar being "addictive" in the same clinical sense as drugs of abuse. It is highly palatable and habit-forming in ordinary ways; the stronger addiction framing is contested among researchers.
- Sugar "feeding" cancer in any way that justifies eliminating it — all cells use glucose, and cutting dietary sugar does not starve tumours.
Practical reductions that stick
- Start with drinks. Replacing sweetened beverages with water, plain sparkling water, or unsweetened tea and coffee is the single largest lever available to most people.
- Step down gradually rather than quitting abruptly. Taste preference for sweetness genuinely adjusts over a few weeks, and slow reduction is far more sustainable.
- Buy plain and add. Plain yoghurt with fruit you add yourself contains a fraction of the sugar of the flavoured version. Same for oatmeal and coffee.
- Do not fear whole fruit. It is not where anyone's sugar problem lives.
- Treat desserts as desserts. Occasional deliberate sweets are not the issue; the constant background sugar in everyday staples is.
On artificial sweeteners: current evidence suggests they are a reasonable transitional tool for reducing sugar-sweetened drink intake, though the WHO has advised against relying on them for long-term weight control. They are one option, not a requirement.
The short version
The target is added and free sugars, not sugar in whole foods. Under about 25 to 50 grams a day depending on which guideline you follow — divide grams by four for teaspoons. Sugary drinks are usually the biggest source and the easiest fix. Whole fruit is fine; juice mostly is not. And honey, agave, and coconut sugar count as added sugar no matter how the packaging is styled.
